Minneola Long Term Care Center

 Minneola Long Term Care center was built in 1976.  The facility was updated in 1995.  Minneola Long Term Care Center consists of 36 beds.  We have both private and semi private roomsMinneola LTC is attached to the hospital via a hallway.

Carol, our Director of Long Term Care has been part of the facility since 1986.  Carol's passion and conviction to quality care provides a wonderful base for which the LTC is built on. 


Jami, our Director of Nursing, has been an RN since 1994.  In 2000 she became certified in gerontology (geriatrics). She has been with the facility since 2001. 



Our home was one of seven facilities in the state chosen by the Kansas Department on Aging as a PEAK award winner in 2006.  The ”Promoting Excellent Alternatives in Kansas” nursing home initiative is designed to recognize homes that have changed the culture of their facility by increasing resident control, empowering front-line staff, enhancing the physical environment, and increasing community involvement.   Our interest in the PEAK initiative began when an anonymous staff member posted a sign in the break room which stated, “Our residents don’t live in our facility—we work in their home.”  That quickly became our motto and has served as our guiding principle in change.  We believe that we are called to provide an environment where residents can direct their care, lead meaningful lives, and maintain their ties with the community at large, and we welcome input from residents and families to help us reach those goals.
